What is an NDIS Complex Wound Assessment?

An NDIS Complex Wound Assessment is a comprehensive clinical evaluation performed by a qualified Wound Care Clinician or Nurse to address wounds that are slow to heal or at high risk of complication due to a participant's disability. This includes pressure injuries, diabetic ulcers, vascular wounds, and traumatic or surgical wounds. The assessment determines the wound's cause, stage, and optimal treatment pathway, focusing on creating a personalised plan to promote healing, prevent infection, and improve overall quality of life.

Our Assessment Process

Step 1: Initial Enquiry & Information Gathering

We begin with a confidential discussion to understand the wound's history, your overall health, disability-related factors, and NDIS goals. This helps us prepare for a targeted assessment.

Step 2: Comprehensive Clinical Assessment

Our wound specialist conducts a thorough examination. This includes measuring and staging the wound, assessing the surrounding skin, identifying underlying causes, and evaluating risk factors like mobility and nutrition.

Step 3: Detailed Report & Recommendations

You receive a comprehensive report with a diagnosis, wound management plan, and product recommendations. This includes the correct wound cleansing techniques, advanced dressings, and pressure-relieving equipment needed.

Step 4: Implementation of the Plan

We assist in sourcing the recommended wound care products and equipment through your NDIS plan. We provide hands-on training to you and your support team on the correct wound care procedures.

Step 5: Ongoing Monitoring & Review

We schedule regular follow-up visits to monitor healing progress, adjust the treatment plan as needed, and prevent recurrence. Consistent monitoring is key to successful wound closure.

What types of wounds require a complex wound assessment?

This assessment is for wounds that are not healing as expected, such as pressure injuries (bed sores), diabetic foot ulcers, venous leg ulcers, surgical wounds that have broken down, and wounds resulting from trauma or poor circulation.

Is wound care covered by the NDIS?

Yes, if the wound is directly related to your disability (e.g., a pressure injury due to immobility). Funding for assessments and ongoing wound care products is typically available under the Capacity Building - Improved Health and Wellbeing or Core - Consumables budgets.

Who performs the complex wound assessment?

Assessments are conducted by our specialised Wound Care Nurses or Clinicians who have advanced training in wound management and understand the unique challenges faced by NDIS participants.

How do I book an assessment with Beyond Health Services?

Booking is simple. Click the "Book Now" button on this page or call our clinic directly. We will discuss your situation, confirm your NDIS funding details, and schedule an assessment with one of our wound care specialists.

What happens during the assessment?

The clinician will gently examine the wound, take measurements and photographs (with consent), assess your general health and mobility, and discuss your current wound care routine to identify any barriers to healing.

Will the assessment be painful?

Our clinicians are trained to be as gentle as possible. The goal is to assess the wound without causing additional pain or trauma. We will communicate each step clearly to ensure you are comfortable

Do you provide the wound dressings and equipment?

We provide the assessment and plan. The recommended dressings and pressure-relieving equipment (like special mattresses or cushions) are then sourced separately through your NDIS funding, and we will guide you through this process

How often will I need follow-up visits?

The frequency depends on the severity of the wound. Initially, visits may be weekly or bi-weekly to closely monitor progress. As the wound improves, the time between visits will increase.

"I’m so grateful for Liam, the driver who takes my father to his medical appointments. Liam is always on time, helps my dad in and out of the car, and even waits with him at the doctor’s office. It’s reassuring to know my dad is in such capable hands when I can’t be there myself."
Adam G, Windsor, Melbourne
"These guys sent over a fantastic speech therapist to help my son with his communication challenges. Kate was incredibly patient, using fun and interactive techniques to get him to open up.
