What is an NDIS Health Risk Assessment

An NDIS Urinary Catheter Assessment is a comprehensive clinical evaluation conducted by a qualified healthcare professional, such as a Continence Nurse or Physiotherapist. The assessment determines if catheterisation is a necessary and suitable continence management strategy for an NDIS participant. It focuses on identifying the right type of catheter (e.g., intermittent, indwelling, suprapubic), the correct size, and providing essential education to ensure the procedure is safe, effective, and supports the participant's dignity, independence, and overall health.

Our Assessment Process

Step 1: Initial Enquiry & Information Gathering

We have a detailed conversation to understand your current situation, health history, NDIS goals, and any specific concerns you or your support network may have.

Step 2: Comprehensive Clinical Assessment

Our clinician visits you to conduct a thorough assessment, reviewing areas like medication schedules, skin integrity, mobility, nutrition, and home environment for potential risks.

Step 3: Detailed Report & Recommendations

You receive a clear, actionable report that outlines identified risks, prioritises them, and provides practical strategies and recommendations to mitigate each one.

Step 4: Implementation of the Plan

We work with you, your family, and support workers to integrate the risk management strategies into your daily routine and NDIS plan.

Step 5: Ongoing Monitoring & Review

We schedule follow-ups to monitor the effectiveness of the plan, make adjustments as your needs change, and ensure continuous risk reduction.

What is the difference between a Health Risk Assessment and a standard medical check-up?

A standard check-up focuses on your current health status (e.g., blood pressure, heart rate). A Health Risk Assessment is forward-looking; it identifies potential future problems (like falls, skin breakdown, or medication errors) related to your disability and creates a specific plan to prevent them from happening. It's about proactive management rather than reactive treatment.

Who should get an NDIS Health Risk Assessment?

This assessment is highly beneficial for any NDIS participant who wants to proactively manage their health, especially those with complex needs, multiple medications, mobility limitations, a history of health incidents, or those living independently with support. It's a key tool for preventing avoidable complications.

What kind of risks does this assessment identify?

We assess a wide range of potential risks, including:

  • Falls Risk: Home safety and mobility.
  • Medication Management: Errors or difficulties with administration.
  • Skin Integrity: Risk of pressure sores/wounds.
  • Nutrition/Hydration: Challenges with eating or drinking.
  • Swallowing Difficulties (Dysphagia): Risk of choking or aspiration.
  • Social Isolation: Impact on mental well-being.
  • Behaviour of Concern: Risks to yourself or others.
How can a Health Risk Assessment help my NDIS plan?

The assessment report provides concrete evidence of your specific support needs related to risk prevention. This evidence is crucial for justifying funding for supports like specialist allied health, assistive technology, home modifications, or increased core supports during your plan review, ensuring your funding is matched to your actual needs.

Is this assessment covered by my NDIS funding?

Yes. A Health Risk Assessment is considered a reasonable and necessary support under the Capacity Building - Improved Health and Wellbeing category. It is designed to build your and your support team's capacity to manage health risks effectively, which is a core goal of the NDIS.

What qualifications do the assessors have?

Our assessments are conducted by experienced Registered Nurses or Allied Health Professionals who have specific expertise in disability and proactive healthcare. They are skilled in identifying subtle risks and creating practical, participant-focused management plans.

Will the assessor just tell me everything I'm doing wrong?

Not at all. Our approach is positive and empowering. We focus on your strengths and work with you to find solutions that make your life safer and easier. The goal is to give you more confidence and control, not to criticise your current situation.

What happens after the risks are identified?

We don't just hand you a report. We create a practical, step-by-step risk management plan. Our team can help you and your support network understand and implement the recommendations, and we offer follow-up monitoring to ensure the strategies are working effectively.

What Families Have to Say

"My support worker, Sarah, has been an absolute lifesaver. She helps with my daily routine, from meal prep to getting to appointments on time, and even takes me on short walks to the park. Sarah is patient and really listens to what I need. I’m so grateful for the support she provides – I feel more confident knowing she’s there."
Emily T, Brunswick, Melbourne
"We hired a gardener from Beyond Health Services, and Jason has completely transformed our backyard. He removed the weeds, trimmed the hedges, and even installed a vegetable garden that my wife and I have been enjoying. Jason M, St Albans, Melbourne
"The cleaning service from Beyond continues to impress us. Every corner of our home gets attention – they even scrubbed out stains from the carpets that we thought would never come out. I always look forward to coming home to a fresh, spotless house after their visit. Their reliability is what sets them apart."
Sophia R, Sunshine West, Melbourne
"I’m so grateful for Liam, the driver who takes my father to his medical appointments. Liam is always on time, helps my dad in and out of the car, and even waits with him at the doctor’s office. It’s reassuring to know my dad is in such capable hands when I can’t be there myself."
Adam G, Windsor, Melbourne
"These guys sent over a fantastic speech therapist to help my son with his communication challenges. Kate was incredibly patient, using fun and interactive techniques to get him to open up. In just a few weeks, we’ve seen a noticeable improvement in his speech, and he’s more confident in expressing himself. We’re so thankful for Kate’s expertise and warmth."
