What is an NDIS Vital Signs Assessment?

An NDIS Vital Signs Assessment is a fundamental health check conducted by our qualified professionals to measure key indicators of your bodily functions. This includes tracking blood pressure, heart rate, temperature, respiratory rate, and oxygen saturation. For NDIS participants, this assessment is crucial as it provides objective data to inform your support needs, track progress towards your health-related goals, and ensure your NDIS funding is accurately aligned with your well-being requirements. It's a proactive step towards maintaining your health and maximising your independence.

What is included in an NDIS Vital Signs Assessment?

An NDIS Vital Signs Assessment typically includes the measurement of core physiological indicators: blood pressure, heart rate (pulse), respiratory rate, body temperature, and blood oxygen saturation (SpO2). At Beyond Health Services, our assessment also includes a review of your overall well-being and how these vitals relate to your specific NDIS goals, providing a holistic view of your health.

Who is qualified to perform an NDIS Vital Signs Assessment?

The assessment must be conducted by a qualified and registered health professional such as a Nurse or an Allied Health professional. At Beyond Health Services, all our assessors are fully qualified, NDIS-approved, and have extensive experience working with participants, ensuring the assessment is both clinically sound and understanding of your NDIS journey.

How can a Vital Signs Assessment help my NDIS plan?

The results provide objective, measurable data that can be used as evidence for your NDIS plan review. It can justify the need for certain supports (e.g., increased core supports for complex health needs), demonstrate progress towards health-related goals, and ensure your funding accurately reflects your current health status, leading to a more robust and effective plan.

Is the NDIS Vital Signs Assessment covered by my NDIS funding?

Yes, if it is a reasonable and necessary support related to your disability. The cost is typically claimed from the Capacity Building budget under "Improved Health and Wellbeing." Our team at Beyond Health Services can help you confirm your funding eligibility before you book.

How long does the assessment take?

The physical measurement of vital signs is quick, usually taking about 15-20 minutes. However, the entire appointment, including the initial discussion, the assessment itself, and explaining the next steps, typically takes between 45 to 60 minutes to ensure we are thorough and you feel completely informed.

Where will the assessment take place?

We offer assessments at a location that is most comfortable and convenient for you. This can be in your own home, at a supported accommodation facility, or at one of our accessible clinics. Our goal is to minimise stress and make the process as easy as possible.

What happens after the assessment?

After the assessment, we create a comprehensive report with clear recommendations. We will discuss this report with you and your support network. We can also assist in communicating these findings to your NDIS planner or Support Coordinator to ensure the recommendations are actioned effectively.

How often should I have a Vital Signs Assessment?

The frequency depends on your individual health needs and NDIS goals. It can be a one-off assessment to establish a baseline, or it may be scheduled regularly (e.g., quarterly or annually) for ongoing monitoring of a chronic condition or to track the effectiveness of new supports. We will advise on a suitable schedule based on your initial assessment.

In their words

What Families Have to Say

"My support worker, Sarah, has been an absolute lifesaver. She helps with my daily routine, from meal prep to getting to appointments on time, and even takes me on short walks to the park. Sarah is patient and really listens to what I need. I’m so grateful for the support she provides – I feel more confident knowing she’s there."
Emily T, Brunswick, Melbourne
"We hired a gardener from Beyond Health Services, and Jason has completely transformed our backyard. He removed the weeds, trimmed the hedges, and even installed a vegetable garden that my wife and I have been enjoying. The space looks so much better, and we couldn’t be happier with his attention to detail."
Jason M, St Albans, Melbourne
"The cleaning service from Beyond continues to impress us. Every corner of our home gets attention – they even scrubbed out stains from the carpets that we thought would never come out. I always look forward to coming home to a fresh, spotless house after their visit. Their reliability is what sets them apart."
Sophia R, Sunshine West, Melbourne
"I’m so grateful for Liam, the driver who takes my father to his medical appointments. Liam is always on time, helps my dad in and out of the car, and even waits with him at the doctor’s office. It’s reassuring to know my dad is in such capable hands when I can’t be there myself."
Adam G, Windsor, Melbourne
"These guys sent over a fantastic speech therapist to help my son with his communication challenges. Kate was incredibly patient, using fun and interactive techniques to get him to open up. In just a few weeks, we’ve seen a noticeable improvement in his speech, and he’s more confident in expressing himself. We’re so thankful for Kate’s expertise and warmth."
