Support Coordination 

Support Coordination is a Capacity Building support funded by the NDIS to help participants understand and implement their NDIS plan. A Support Coordinator works with you to connect with services, manage funding, build your skills, and achieve your goals. They act as a guide through the NDIS system.

Support Coordination is not automatically included in every NDIS plan. It's typically funded if you have:

• Complex support needs
• Limited informal supports
• Recently transitioned to the NDIS
• Significant barriers to accessing or coordinating supports

You can request it during your planning or review meeting and explain how it will help you use your plan more effectively.

A Support Coordinator can help you:

• Understand your NDIS plan
• Link you with providers that meet your needs
• Build your confidence to manage your plan independently
• Coordinate service bookings and agreements
• Resolve issues with providers or services
• Prepare for plan reviews

They ensure your supports are working well together and aligned with your goals.

• Support Coordination helps you find and connect with services.
• Plan Management handles the financial side of your NDIS plan, such as paying invoices and keeping track of your budget.

You can have both services in your plan if they’re funded.

There are three levels of Support Coordination:

• Support Connection – short-term help to connect with services
• Support Coordination – ongoing assistance to implement and manage your plan
• Specialist Support Coordination – for participants with complex needs and high-level risks, requiring specialised intervention

Most participants receive the mid-tier level: Support Coordination.

No. Support Coordination must be requested and justified. You need to explain:

• Why you need help managing or implementing your plan
• If you lack informal supports (like family or friends who can assist)
• If your situation is complex or changing frequently

The NDIS will consider these factors when deciding whether to include it.

Yes. You can choose your Support Coordinator based on your plan type:

• If you're self-managed or plan-managed – you can use registered or unregistered providers
• If you're agency-managed – you must choose an NDIS-registered provider

Make sure the coordinator understands your needs, listens to you, and communicates clearly.

Your Support Coordinator is doing a good job if they:

• Keep you informed and involved in decisions
• Help you make progress towards your goals
• Are responsive and reliable
• Regularly check in with you and adjust supports as needed
• Empower you to become more independent over time

If you're not happy with your Support Coordinator, you can change providers at any time.
